Welcome patients new and established! We look forward to seeing you soon in our office. Below are a couple of ways to help expedite the paperwork process and make your appointment a pleasant one.
If you are a new adult patient, please see Patient Forms on the right side of the web site and click on the individual forms listed (all except the pediatric questionnaire) and print them. Take your time and fill them in as complete as possible. If you mess up, don't worry, just reprint them and complete the new ones. We are not attempting to pry into your personal life but there are certain questions that we must ask in order to have the physician treat you appropriately. Other questions on these forms are required for administrative/billing reasons. It is required that ALL questions and blanks are filled in completely. If you need assistance in filling out these forms, please bring them in before your appointment and one of our staff members will be more than happy to assist you in private to avoid any embarrassing situations.
If you are a parent of a minor (under 18 yrs. old), please follow the directions above but include the pediatric questionnaire. These forms need to be completed by the parent/legal guardian and keep the answers about the minor. Parents/Legal guardian MUST sign all forms.
Please make sure that when you come in for your first appointment you bring in the forms, any and all medications including any over-the-counter medications.
Familiarize yourself with the location of our office (see Directions on this site).
Arrive 15-20 minutes in advance of your appointment so that we can verify your insurance benefits and enter your personal information into the computer.
Based on your insurance, you will be required to pay your portion or co-pay at the reception window at the time of checking in. Medicare patients will need to pay at the check out window prior to making their follow-up appointment. If you are unable to pay at the time of your appointment, please contact our office ahead of time so that we can reschedule your appointment at a time when it is more convenient for you.
We try our best to lessen the wait time that you have in our office. Please return the courtesy by contacting our office 24 hrs. in advance if you cannot keep your scheduled appointment. If your condition changes prior to your appointment, please contact the office as soon as possible and we will try to accommodate you and get you in earlier if it is deemed necessary.
